As a member of Liberty Medicare Advantage Plans you are entitled to receive a list of all providers (Provider Directory) and pharmacies (Pharmacy Directory) that are in the network. You are also entitled to receive a list of covered drugs (Formulary). If you would like a formulary, provider or pharmacy directory mailed to you, you may call 1-844-854-6884.
